Simply work through the 2 sections under the heading 'Commit'. If you answer 'no' to a question then use the guidance notes, examples and templates to help you.
There is a template available for each document that you need to submit - customise, brand etc these however you wish. You can create your own documents if you prefer, but make sure you read the requirements of the Standard carefully to ensure your work complies.
